Prototype Sites

NHS Improvement have recently appointed seven national prototype test communities to implement and evaluate whole pathways of care for patients living with or beyond a diagnosis of either lung, breast, prostate or colorectal cancer.  These prototype sites will be using stratified pathways within a three level model of care and support.

This new phase builds on the testing work so far and introduces stratified pathways based on a patient’s disease, their treatment and on them as an individual.

 

Two enabling projects, remote monitoring and care coordination, are seen as essential to the testing of the stratified pathways, as is access and availability of information prescriptions and patient support services.  A small group has been established to lead the remote monitoring work and the presentations from their first workshop event are available to download below.
