Screening and Assessment Tools
The tools below have been tested for use with people affected by cancer and found to be useful screening and assessment tools.
The NCSI aims to establish the best way for health professionals to assess the ongoing needs of cancer survivors.
The tools below have been tested in the Assessment and Care Planning Test Communities and have proved to be useful for screening and assessment;

At the end of testing, 10 out of 11 sites were using the Distress Thermometer assessment tool. This tool was revised during testing to include lifestyle assessments and was renamed as the Concerns Check List – available to download below. This revised version will be tested further in some of the 2011 test communities.

Use of the term and symbol ‘Distress Thermometer’
We thought it would be useful to flag the discussion with NCCN when the Assessment and Care Planning project manager applied for a licence to use the Distress Thermometer (DT).
There are three elements to the Distress Thermometer:
- the title ‘Distress Thermometer’,
- the thermometer symbol
- the text as outlined in the original or the adapted version by James Brennan.
Our understanding is that the owners of the copyright the NCCN do not allow you:
- to change the title and still use the thermometer symbol with the text
- to use the title without the thermometer
- to use the DT without licence from the NCCN http://www.nccn.org/index.asp
- to use online, unless you stipulate that you are using on line when applying for the licence.
Note: They do allow you to change any of the text
